RELEASE OF LIABILITY WAIVER

I agree to hold harmless Main Street Baseball, Inc. its officers, board members, sponsors, coaches, supervisors and volunteers for any claim arising from accidental injury to my player. The insurance policy of Main Street Baseball, Inc. will be considered secondary to any existing insurance. I understand that in my absence, emergency medical treatment may be administered to my player until such time I can be contacted. I agree to the terms and conditions of this liability release and give my permission for my player to participate in this activity.

• I have read and understand the contents of this waiver and release of liability. I represent that I am authorized to execute and submit this application on behalf of the minor participant.

• I agree, represent and affirm that the minor participant is in good health and good physical condition and does not suffer from any disability that would prevent or limit their voluntary participation in clubs/leagues, clinics, lessons, open play, related activities and special events.

• I agree that I, as parents and/or legal guardians, will instruct the minor participant that prior to participating, he or she should become or be familiar with the operation of, and inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he should immediately advise his coach or supervisor of such conditions and refuse to participate.

• I assume all the foregoing risks and accept personal responsibility for any damages or losses as a result of injury, permanent disability, death or loss of property.

• I release, waive, discharge, indemnify, hold harmless and covenant not to sue Main Street Baseball, Inc. its members, administrators, directors, agents, coaches, and employees, or other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessor of the premises, from and against any and all losses, claims, damages, or expenses, including, without limiting the generality of the foregoing, attorney’s fees, arising from the named child’s participation in club/leagues, clinics, lessons, open play, related activities or special events.
